Accepted Insurances
At this time, we are in the process of credentialing and are able to see the patients with the following insurances.

Credentialed with Medicare, Medicaid, Cigna, Humana, United Health Care, Aetna, Ambetter, Amerigroup, Community Health Choice, Molina, Tricare PPO, BCBS
Credentialing in process for:
Ambetter, Wellcare, Tricare HMO,
Patients who prefer to self pay (cash, check, credit card)
We also offer discounted pricing program "Affordable Specialist Care Program" for our "Self Pay" patients who do not have insurance.
The pricing for in person or telehealth visit are as follows:
New patient: $400 for 30 min face 2 face time (comes with complimentary additional 15 min of documentation time and non face to face 15 min of record review time.)
Follow up patient: $300 for 30 min face 2 face time
Large joint injections $150 (includes medication cost)

Meditate and Unwind with a doc (VIrtual Session)
Meditation is the key to Mind and Body Awareness
Meditate and unwind is a program offered once a month to allow a space to slow down, calm down, and rejuvenate. It creates a space in our busy lives to feel ourselves and become aware of the very life within us.
It includes 15 min introduction, and 30 min meditation and is followed by 15 minutes of reflection time. Ask our staff about our virtual 'Meditate and Unwind with a Doc' program.

You are welcome to select a lab or imaging center of your preference. By default, our orders are electronically transmitted to Quest for labs and Memorial Hermann Imaging Center for imaging. These establishments typically furnish us with electronic results. Should you opt for an alternative lab or imaging center, please provide them with our fax number, 314-405-9678, and request that they transmit the results to us via fax.

Medication Refill Disclaimer
As a policy, we exclusively prescribe or refill medications during patient visits, except in urgent scenarios. If you require a medication refill, kindly contact our office at 409-276-5595. Please note that we do not accept refill requests from pharmacies; these requests are exclusively accommodated by our patients to prevent prescription errors.
ADA Disclaimer
We regret to inform you that we are unable to complete the Disability Accommodation Request Resolution Form (ADA) on your behalf. We kindly request that you reach out to your PCP (Primary Care Physician) and request them to fill out the form. Nevertheless, we are more than willing to facilitate the transmission of your medical records to your PCP's office if they should require them.
